Transcript Request

Transcripts may be requested in the following ways:
•In person
•By mail

In person:
Transcript request forms can be picked up in the Registrar’s Office. Once the form is filled out, it must be taken to the Business Office for approval. If a balance exists only an unofficial transcript can be released.

By mail:
Request should include the following:
•Print out a copy of the transcript request form, click here to download.
•Fill out the form accordingly. Include all asked information.
•The address to which the transcript is to be mailed; and your signature.
(Your request will not be processed without your signature)
•Payment by check or money order should accompany your request.
